Good News about the Vision Care Plan!

Davis Vision, administrator of the Vision Care Plan for the CWA WV VA (142) bargaining unit, has made a provider network modification that increases the number of in-network providers nationwide.  The upgrade to the network is in place now.

To find a provider, Plan members can use the Davis Vision Provider Locator tool via the Davis Vision website—either by logging into their private account, or by using the Frontier client code “4849”.


  1. Go to www.davisvisioncom/members
  2. Click on “Member Log In”
  3. Log In: Member can input their username and password if they’ve already set up an account, or can Register New Account if they have not previously done so.
  4. Search anonymously by Client Code: Input 4849 in the “Client Code” space which will provide an anonymous log in for the Frontier Plan and Network.
  5. Lastly, click on “Find a Provider” and input zip code & radius(miles) for search

To use the provider network, Plan members should simply call the provider to make an appointment and provide their Davis Vision ID#.  The provider will verify their eligibility directly with Davis Vision.
